Living and Moving with Confidence!

Sapphire Rehab physical therapists are among the most highly trained and competent in the industry. We cater to all client demographics, have the support of a Quality Assurance and Professional Development team, and evidence-based practice & equal clinical excellence with interventions.

Sapphire Rehab physical therapists assess and address the varied components of patients’ functional mobility and work to improve their quality of life. Treatment may be provided in our clinic, area-specific, or in the comfort of the patient’s home environment-specifically addressing functional impairment that resulted in difficulties in activities of daily routine.

Sapphire Rehab PTs take functional wellness to the next level. These health professionals administer exercise stress tests to evaluate our patient’s overall health, with special attention to cardiovascular function and metabolism. Based on the results, they develop individualized exercise prescriptions to increase physical strength, endurance, flexibility, and balance. These programs help patients with heart disease and other chronic conditions, like diabetes, pulmonary (lung) disease, peripheral vascular disease, and hypertension, to regain their health.

Physical therapy services can be provided individually in the home or in a community-based group setting.

It is a condition where a person experiences a spinning, lightheaded, or unsteady sensation.

Muscle
Weakness

A person’s muscles lack the strength or power needed for proper function and mobility.

Falls Risk and
Fear of Falling

Simultaneous consideration of two interrelated aspects of an individual’s well-being.

Limitation or
Loss of Function

Physical limitations can hinder one’s ability to perform certain tasks.

How can we help

What are the benefits of physical therapy?

Decrease Pain

Comprehensive physical therapy treatment plans could help alleviate and reduce common etiologies of pain and increase muscle strength flexibility, improve joint stability, and blood circulation.

Reduce Fall Risk

Sapphire Rehab physical therapists employ customized exercise programs proven to decrease the risk of falls especially on older adults.

Increase speed of Functional Mobility

Walking speed has been called the “Sixth Vital Sign” because of its importance and relevance to older adults’ functional mobility and mortality. Sapphire Rehab Physical Therapists intervene with treatments to increase walking speed, and improve safety and stability. Evidence indicated that increasing walking speed by as much as 0.1 meters/second not only improves functional independence, but it also increases life expectancy. (Studenski et al, 2011)

Improve Balance

Improving balance and reducing the risk of falls begins with an extensive, patient-specific functional assessment. Evaluating patients in the clinic, in simulated environments and even at their own homes allows Sapphire Rehab Physical therapists to accurately identify factors that could to falls. Proper prescription of therapeutic exercise and balance retraining, would help reduce balance-related issues, falls and injuries.

Compensatory Training

Compensatory training, is prescribed when an area of limitation is off limits to loading, weight training (fractures, poor exercise tolerance, poor postural alignment). This approach is geared on strengthening/improving viability of affected parts - to allow improvement in safety, balance & functional mobility, while allowing affected body regions to recover & be injury free. Compensatory training is subject to upgrades & adjustments as necessary, in order to promote a return to normal/near normal function.

Improve General Safety & Well Being

Sapphire Rehab PT's attentively screens clients and meticulously gathers information on changes including overall balance and how pathology, impairments, functional limitations, behavioral patterns, and environmental factors affects the patient overall ability to move and execute normal functional mobility skills. If the individual demonstrates signs outside normative values, we request physician’s orders for a bespoke treatment program - to prevent an adverse events and injuries.

Assistive Device Assessment

With changes in function due to weakness, impaired mobility/balance and at time injury/loss of a body part - Assistive Device Assessment and prescription might be necessary. Sapphire Rehab Physical Therapists help clients thru proper assessment based on the need & facilitate device provision via collaboration with providers & payors.

Balance and Falls

A big part of treating balance and reducing fall risk is related to strength. So, when we do therapeutic exercises properly, we reduce balance-related issues and falls. Sapphire Rehab Therapists proactively screen patients and gather information on changes, including overall balance and how pathology, impairments, functional limitations, behavioral patterns, and environmental factors affect the ability to move in space. If the individual demonstrates signs outside normative values, we request a physician’s orders for a tailored treatment program before an adverse event occurs.

Differential Diagnosis

Differential diagnosis means “distinguishing a particular disease or condition from others that present with similar symptoms.” Since we see patients in their home setting, this is a critical skill sharpened with frequent education from our Quality Assurance and Professional Development Division.

Paradigm Rehab clinicians intensely study the functional impairment and then, following Medicare guidelines, send their differential diagnoses, evaluation, and plan of care to a physician for sign-off. We do not diagnose medical diseases. So when “red flags” indicate a condition that falls outside our scope, we refer to the appropriate medical professional to ensure patient safety and improvement.

For physiological monitoring, monitor vital signs, including respiratory rate, heart rate, lung sounds, blood pressure, and oxygen saturation throughout sessions, treatments, and interventions to ensure that the therapeutic plan of care is properly dosed and the risk of adverse events is mitigated.
